Gather as much information as you can before beginning your on-line claim report (or before reporting your claim by telephone), but do not delay report a claim simply because you are missing a piece of information. All claims should be reported to MEMIC within 24 hours of your notice or knowledge of the injury. It is especially important that you provide us with the correct policy number. For this reason, when you make your report on-line, you will be asked to verify that it is correct.

You should report the claim as soon as is possible (after taking care of the immediate emergency if there is one, of course). By Telephone: You can also report your workers' compensation claims by dialing 1-800-MEMICWC. We'll complete a first report of injury form for you. When you call, you will be given a claim number and MEMIC will begin handling the claim.

Once you have completed the online form and clicked the "Send Report" button, you will see a screen that will confirm that your report has been submitted to MEMIC. Please record the tracking number on the screen for future reference. Once your claim has been assigned a number, the number will automatically be sent to you via e-mail provided you give us an email address on the form. If you do not provide an e-mail, you will receive a paper copy of the report by regular mail within a few days.
