What information do I need to file a report?
Gather as much information as you can before beginning your on-line claim report (or before reporting your claim by telephone), but do not delay report a claim simply because you are missing a piece of information. All claims should be reported to MEMIC within 24 hours of your notice or knowledge of the injury. It is especially important that you provide us with the correct policy number. For this reason, when you make your report on-line, you will be asked to verify that it is correct.

When Should I Report an Injury?
You should report the claim as soon as is possible (after taking care of the immediate emergency if there is one, of course). All claims should be reported to MEMIC within 24 hours of your notice or knowledge of the injury. Do not delay reporting a claim if you are missing a piece of information.
Do I have to report online?
You are not required to report the injury online. You may also report by telephone. We recommend that you report the injury in the most expedient and comprehensive manner possible.

By Telephone: You can also report your workers' compensation claims by dialing 1-800-MEMICWC. We'll complete a first report of injury form for you. When you call, you will be given a claim number and MEMIC will begin handling the claim.

How do I know if my injury report has been received if I file it online?
Once you have completed the online form and clicked the "Send Report" button, you will see a screen that will confirm that your report has been submitted to MEMIC. Please record the tracking number on the screen for future reference. Once your claim has been assigned a number, the number will automatically be sent to you via e-mail provided you give us an email address on the form. If you do not provide an e-mail, you will receive a paper copy of the report by regular mail within a few days.
 
PLEASE NOTE: For the sake of security, if you do not finish the online report by clicking on the "Submit Form" button or "Submit and Print" button within 45 minutes of starting it, the information you have entered on that report will be lost.
